As more organizations move toward a model based on reimbursement for results, rather than procedures, finding ways to reduce readmissions is exceedingly important, as one in five people are readmitted to the hospital within 30 days of discharge. Reasons for readmission vary, but poor care transition, care program compliance and the lack of an ability to constantly monitor the patient’s condition are major contributing factors. Better care coordination, case management and increasing the follow-up “sample rate” are ways to reduce readmission rates and are especially important for those organizations moving toward a patient-centered medical home or Accountable Care Organization model.

The platform allows caregivers to discharge patients earlier, thus decreasing the chances of contracting a Hospital Acquired Condition, while at the same time enabling close monitoring of the patient’s recovery. The platform allows the health provider organization to perform better patient progress monitoring once they are discharged by performing follow-up care via virtual visits. This enables caregivers to perform more frequent, short follow up visits, which would not be practical in an office setting, and reduces discomfort and potential harm to the patient by allowing them caregiver access from home, rehab or assisted living facilities. The end result is that post-hospitalization complications are caught earlier, readmissions are reduced and patients receive a higher standard of care.
